About the role

Overview

Join our dedicated home health team as a Home Health Director of Nursing (DON), where you will lead and oversee nursing services within a dynamic home health agency. This vital role combines clinical expertise with leadership excellence, ensuring the delivery of exceptional patient care while maintaining compliance with all regulatory standards. As a key leader, you will manage nursing staff, coordinate care plans, and uphold the highest standards of quality and safety in a fast-paced, compassionate environment. Your expertise in long-term care, hospice, assisted living, and acute care settings will drive positive outcomes for our patients and support the growth of our team.

Duties

  • Provide clinical leadership and supervision to nursing staff, including nurse managers, case managers, and home health nurses.
  • Develop, implement, and review individualized care plans that meet each patient’s unique needs, ensuring compliance with Medicare, Medicaid, and other regulatory requirements.
  • Oversee documentation review and ensure accurate medical records management using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems.
  • Monitor quality assurance processes to maintain regulatory compliance with long-term care regulations and HIPAA privacy standards.
  • Manage staffing schedules, shift management, and resource allocation to ensure optimal patient coverage and staff satisfaction.
  • Conduct regular audits of medical records, documentation review, and care plan updates to uphold high standards of patient care.
  • Lead efforts in staff training on medical terminology, physiology, anatomy knowledge, and regulatory updates to foster continuous professional development.
  • Collaborate with interdisciplinary teams to coordinate services across assisted living facilities, nursing homes, hospice care providers, and home health agencies.
  • Ensure effective communication with patients’ families regarding care plans and progress while promoting a culture of compassionate care.

Requirements

  • Proven experience in assisted living facilities, nursing homes, or home health environments with a strong background in long-term care or hospice care.
  • Demonstrated supervisory or nurse management experience within a healthcare setting.
  • In-depth knowledge of Medicare regulations, long-term care regulations, and regulatory compliance standards.
  • Familiarity with EMR/EHR systems for documentation review and medical records management.
  • Strong understanding of medical terminology, physiology, anatomy, and patient care best practices.
  • Experience managing care facility teams with leadership skills in nursing administration and clinical supervision.
  • Excellent communication skills to effectively lead staff teams and engage with patients’ families.
  • Ability to handle shift management responsibilities efficiently while maintaining high-quality patient outcomes.
  • Knowledge of HIPAA privacy rules and commitment to maintaining confidentiality at all times.
