Home Health Consultant

About This Role

This Home Health Consultant role involves partnering with field staff and leaders to provide support to patients. The consultant will maintain relationships with existing referral sources and actively prospect for new ones, ensuring alignment between demand for clinical care and agency capacity. The role suits a compassionate individual who can educate various healthcare facilities and physicians on the company's service offerings.

Responsibilities

• Maintain relationships with all existing referral sources
• Actively prospect for new referral sources based on the Agency’s scope of service
• Work closely with Marketing, Agency Administrator, and Clinical Management to ensure that the demand for clinical care is aligned with capacity and at no time compromises the Agency’s ability to provide the best quality care for patients
• Educate referral sources on the Home Health, Journey, Palliative, and Hospice offering, including hospitals, skilled nursing facilities, assisted living facilities, independent living facilities, and physicians

Requirements

• Associate's Degree, Bachelor's Degree preferred
• One year of healthcare marketing experience preferred
