Role Summary The OP Home-Based Physical Therapist provides comprehensive physical therapy services, including assessments, treatment planning, and therapeutic interventions, adhering to professional and ethical standards. They contribute to program development, quality improvement, and interdisciplinary problem solving, ensuring high-quality patient care.
Key Responsibilities
- Conduct evaluations and deliver treatments as prescribed by physicians for patients of all ages.
- Administer therapy techniques such as muscle strengthening, gait training, ADL training, and manage orthotic/prosthetic devices.
- Perform initial and ongoing assessments, document progress, and develop treatment plans with measurable goals.
- Create accurate, timely clinical documentation in compliance with guidelines.
- Educate patients and caregivers on activity routines and safety measures.
- Communicate with healthcare team members, updating on patient progress and coordinating care.
- Manage scheduling, transportation, and portable therapy equipment efficiently.
- Maintain departmental policies, safety, and quality standards, and participate in program development.
- Monitor patient insurance authorizations and address scheduling or coverage concerns.
- Perform additional duties as assigned.
Qualifications
- High School Diploma or GED required; Doctor of Physical Therapy (DPT) after 2015.
- Licensed Physical Therapist and valid driver’s license required.
- Auto liability insurance and BLS certification within 90 days.
- Must have reliable transportation for the role.
