Health Information Management Technician II

About This Role

This Health Information Management Technician role is responsible for the accurate and complete care of patient health records, ensuring information is accessible only to authorized personnel. The position involves understanding medical terminology and healthcare business lines, assisting caregivers and providers, and handling information in various media forms. It suits a detail-oriented individual with knowledge of health information systems.

Responsibilities

• Oversee active medical records requiring completion
• Evaluate record completion using medical records practice standards for various healthcare facilities
• Coordinate record completion when issues require resolution
• Identify documents constituting the legal content of the patient record in electronic and paper formats
• Assign appropriate document names
• Scan paper health records (internal and external sources) to the target location of the medical record
• Evaluate incoming fax communications, identify request types, and route to appropriate teams
• Upload documents
• Enter specific laboratory or healthcare data into the electronic health record
• Complete electronic record corrections or scanned record corrections accurately according to established guidelines
• Ensure documentation of requests for information within 3 business days of receipt
• Document completion of all requests for information per Corporate Responsibility and HIPAA guidelines
• Receive and validate telephone, verbal, and written requests for information from healthcare providers, authorized third parties, patients, or their authorized representatives
• Follow patient's requests for records, including desired format (electronic, Epic Care Link, paper, or other media), and complete within 30 days
• Keep patients informed of delays
• Complete final patient requests within regulatory timeframes
• Understand and accurately administer the Record Retention schedule
• Accurately identify records that have met the retention schedule
• Assist with tasks to store, retain, and purge patient records
• Perform medical record transcription
• Process birth certificates
• Merge duplicate patient records
• Resolve patient overlays
• Manage chart corrections
• Use all chart correction tools
• Manage Care Everywhere links and incoming messages
• Accurately update Epic with patients' date of death
• Work with the Privacy Officer for breach notifications
• Review conflicting patient data for potential fraud/false identity
• Perform scanned document corrections
