Health Coach

About This Role

This role involves empowering individuals to make long-term behavioural changes to prevent or reverse Type 2 Diabetes by delivering quality healthy lifestyle programmes. The position is for a passionate Health Coach to join the bank team on a casual contract basis, providing county-wide coverage of the NHS Diabetes Prevention Programme in Cornwall.

Responsibilities

• Deliver evidence-based interventions focusing on diet, nutrition, physical activity, and behaviour change
• Facilitate group intervention sessions to support clients in achieving their goals and cultivate a safe and positive environment
• Promote the Healthier You NHS Diabetes Prevention Programme internally and externally, and improve referral pathways between other services and partner organisations
• Engage in ongoing training relevant to your role, such as Behaviour Change Level 1 and 2
• Work towards criteria and targets established within the National Diabetes Prevention Programmes (NDPP) quality assurance framework, including all Clinical Governance procedures
• Work with colleagues across all Thrive Tribe services to contribute to new projects and service developments
• Ensure due consideration and adherence to internal Thrive Tribe policies, including Information Governance, Safeguarding and Complaints procedures
• Help maintain the ethos of Thrive Tribe by driving organisational culture forward across services and embedding values

Requirements

• Experience delivering group-based interventions
• Understanding of the impact personal characteristics and circumstances can have on health behaviours
• Car owner essential
• Right to work in the UK

Nice to have:

• Qualification in a health promotion related subject, such as nutrition, exercise, or health coaching

Important Details

• Schedule: varied and flexible, including evenings and/or weekend work
• Contract: Zero hours contract; 13 sessions over a 9-month period, ordinarily delivering multiple sessions per week/month
• Locations: some home working, with regular travel across Cornwall. Specifically looking for coverage across the West and South of Cornwall, and the Isle of Scilly.
