Health Coach

About This Role

The Admin Health Coach supports family caregivers and their care recipients using empathy, technology, and proven care methods. This role involves dedicated support, education, and guidance to caregivers, providing them with tools and confidence to enable better care for loved ones at home. The Admin Health Coach typically manages more complex, higher-risk cases across multiple states or programs within the Coaching team.

Responsibilities

• Engage, motivate, coach, and educate caregivers via phone and digital technology
• Empower caregivers to manage their own psychosocial, physical, and environmental health to improve and maintain wellbeing
• Manage a caseload and leverage technology to enable effective coaching and information referral
• Complete Semi-Quarterly Home Visits as required by Indiana State regulations
• Provide 8 in-person in-services to each caregiver on their caseload per year as required by Indiana State regulations
• Assess caregiver and care recipient needs, assisting the caregiver in creating and achieving care plan goals; may include referral to external resources
• Collaborate with key stakeholders to assess care/coaching plans and caregiver engagement; identify problems and solutions
• Document and report on coaching activity with a focus on quality and outcomes; make recommendations as appropriate
• Perform case conferences, summary reports, and incident management in compliance with applicable state and program regulatory and/or accreditation requirements
• Solicit feedback from caregivers to identify opportunities for additional support and process improvement
• Perform other duties as assigned

Requirements

• 2 years’ experience in case management, experience working with elderly and disabled
• Exceptional active listening and communication skills with strong empathy
• Tech savvy with comfort using (and teaching others to use) mobile apps
• Ability to multitask in a multi-system environment; proficiency with Microsoft Office required
• Thrives in a fast-paced, start-up environment
• Solid understanding and experience in care planning/case management is required
• Ability to drive up to two hours from home base for client visits
