About the role
Role: Registered Nurse providing in-home patient care within regulatory and agency guidelines.
- Responsibilities: Administer physician orders, develop and update care plans at least every 60 days, document patient progress comprehensively, serve as case manager coordinating multidisciplinary care, participate in quality assurance, and ensure timely, professional documentation.
- Qualifications: Active NYS RN license, NYS driver’s license, CPR/BLS, COVID vaccination, with preferred experience in homecare, acute, or rehab settings. New graduates are welcomed.
- Skills & Attributes: Strong documentation skills, knowledge of Oasis assessments is a plus, and ability to work independently in a home environment.
