Full-Time Physical Therapist

Full-Time Physical Therapist (PT) needed for home health care, offering a $15,000 signing bonus. Role Overview Provide comprehensive physical therapy services to patients in their homes, facilitating recovery from illness or injury. Responsibilities include performing initial and ongoing assessments, evaluating mobility, strength, and function, and developing personalized plans of care. Deliver therapy following physician orders, document progress accurately, and modify treatment plans as needed. Conduct discharge planning and educate patients and caregivers on home exercise programs. Collaborate with healthcare team members, including physicians and nurses, to coordinate care. Supervise personnel such as PTA and HHA where applicable and complete OASIS assessments within scope of practice. Adhere to policies, regulations, and licensing standards, ensuring effective communication with staff, patients, and families. Serve as a clinical resource for team members.

Qualifications & Skills

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted state license as a Physical Therapist.
• Valid CPR, driver’s license, and proof of automobile insurance.
• Preferred: Experience in home health therapy and OASIS documentation.

Key Duties

• Assess and evaluate patient needs and progress.
• Create and update individualized treatment plans.
• Deliver physical therapy services per physician orders.
• Document clinical findings and modify care plans accordingly.
• Coordinate with healthcare team and supervise staff as needed.
• Ensure compliance with regulations and documentation standards.
• Participate in discharge planning and patient education.
