Job Description Seeking a compassionate Palliative Care Nurse Practitioner to provide advanced, patient-centered care in home settings.

Responsibilities Include

Performing comprehensive assessments and developing individualized care plans

Managing treatment adjustments and prescribing medications within legal scope

Maintaining accurate documentation and effective communication with healthcare team

Collaborating with the care team and participating in call rotations

Facilitating referrals and ensuring quality care delivery

Qualifications And Skills Master's in Nursing and Nurse Practitioner licensure with a minimum of two years' experience

Strong decision-making, communication, and leadership skills

Knowledge of palliative medicine, home health, hospice, and relevant regulations

Proficiency with healthcare technology and software

Licenses and certifications including DEA, Part B Provider, and state-specific requirements

Additional Requirements Valid driver’s license and insurance
