About the role
Join as a full-time Registered Nurse in home health, providing personalized, one-on-one care in patients' homes to promote recovery, safety, and independence. Key Responsibilities Include
- Conducting comprehensive in-home assessments to develop patient-centered care plans.
- Providing skilled nursing interventions such as wound care, infusions, medication management, and post-operative care.
- Educating and empowering patients and caregivers for effective symptom and medication management.
- Monitoring patient progress, identifying early changes, and communicating with the interdisciplinary team to adjust care as needed.
- Coordinating care across providers to ensure continuity and high-quality outcomes.
- Completing accurate documentation to support compliance and quality metrics.
Qualifications And Skills Include
- An active RN license in the practice state.
- At least one year of clinical nursing experience.
- Patient-focused, organized, and motivated to deliver high-quality care.
- Reliable transportation, a valid driver’s license, and CPR certification (or ability to obtain).
