About the role
Position Overview: A full-time Physical Therapist (PT) is needed to provide home health care services, enabling patients to recover in their own homes. This role involves assessing patients' mobility, strength, and function, developing individualized plans of care, and delivering prescribed physical therapy interventions. The therapist documents patient progress, modifies treatment plans as needed, and collaborates with healthcare teams, families, and caregivers. Supervising other personnel and completing assessments like OASIS where permitted are key responsibilities. Ensuring compliance with policies, licensure, and documentation standards is essential.
Qualifications & Skills
- Degree in Physical Therapy from an accredited program
- Current, unrestricted state license
- Valid CPR certification
- Valid driver’s license with proof of insurance
Preferred Experience: Home health therapy experience and proficiency with OASIS documentation.
