Field Remote Nurse Practitioner Physician Assistant

About This Role

This field remote role is part of the Jump Start Assessment (JSA) program, a physician-led and advanced practice clinician-driven initiative focused on caring for and supporting Alignment Healthcare patients with complex and chronic care needs. The role involves working within an interdisciplinary team to ensure proper delivery of clinical and home-based patient care, assessing, developing, and coordinating services to promote quality and cost-effective care for seniors aged 65 and over.

Responsibilities

• Conduct in-home assessments (with some outpatient clinical work) on health plan members two days a week
• Work from home three days a week
• Obtain comprehensive history, physical exam, medication review, and appropriate cognitive/depression/safety screenings during in-home assessments
• Be comfortable with end-of-life care discussions
• Collect laboratory specimens in the home setting when appropriate
• Identify diagnoses for active medical management and care management of patients, focusing on chronic disease management
• Communicate patient assessment findings to inform the PCP of potential gaps in care
• Provide patient/family/caretaker education, with an emphasis on close monitoring and follow-up of patient needs
• Emphasize knowledge of appropriate community resources for referral
• Comply with all HIPAA regulations and maintain security of protected health information (PHI)

Requirements

• Master’s degree from an accredited NP Program or PA program
• Ability to communicate positively, professionally, and effectively with others
• Effective problem-solving, organizational, and time management skills
• Ability to work in a fast-paced environment
• Active California state Nurse Practitioner or Physician Assistant license
• Active Nurse Practitioner Board Certification/Physician Assistant Certification
• NPI Number
• DEA
• Valid BLS
• Valid California driver license and current automobile insurance

Nice to have:

• One (1) year of prior clinical or home care experience
• Previous EMR experience
• Experience in care of older adult (geriatric) patients

• Schedule: Monday-Friday, with two days of in-home assessments and three days working from home
