About the role

The SDOH Navigator role is a direct service patient-supporting role, with some administrative responsibilities. The SDOH Navigator supports patients by phone and works with clinical care team members across sites. As part of their role, the SDOH Navigator screens Erie patients and their families for social drivers of health needs. The SDOH Navigator links Erie patients to resources within Erie and in the community. The SDOH Navigator enrolls patients in benefits, schedules medical appointments, and navigates social drivers of health resources such as rental and utility assistance, food assistance, medical transportation, as well as others.

Member Duties : Screens clients for needs around social drivers of health either via telephone or in-clinic and connect to internal and external resources. Provides linkage to services such as emergency financial assistance, transportation, food assistance, etc. Enrolls patients in services, benefits, and programs for which they are eligible (pharmaceutical programs, LINK, financial applications for hospital referrals, early intervention support, DME, urgent referrals etc.). Assists patients in digital navigation, including MyChart enrollment, setting up telehealth capabilities, and other digital assistance related to health. Educates and empower patients to navigate future SDOH needs. Processes internal referrals to the Patient Programs Department. Supports short-term agency initiatives related to SDOH, as they arise.
