About the role
Entry-Level Medical Coder (Brazil) — Remote, Full-Time Join Rex.zone to support global healthcare revenue cycle management by translating clinical documentation into accurate, standardized codes for clean claims and compliance-ready records.
Responsibilities
- Review clinical notes, operative reports, and discharge summaries; assign accurate ICD-10-CM, CPT, and HCPCS codes
- Apply payer policies, NCCI edits, and medical necessity rules to reduce denials and rework
- Validate demographics, provider documentation, and encounter details needed for clean claim submission
- Flag documentation gaps (specificity, laterality, severity) and query providers per policy
- Support charge capture, claim quality checks, and pre-bill edits using EHR and coding tools
- Meet productivity and quality targets while following SOPs and audit standards
Qualifications
- 0–2 years of medical coding or RCM experience (training, internship, or coursework acceptable)
- Working knowledge of ICD-10-CM and basic CPT/HCPCS concepts
- Familiarity with EHR workflows and basic healthcare billing
- Ability to follow coding guidelines, document decisions, and meet turnaround times in a remote setting
Preferred
- Exposure to outpatient/physician, inpatient, or ER coding workflows
- Understanding of claim lifecycle basics (scrubbing, submission, payment posting, denials)
- Experience with QA feedback loops and audit remediation
