About This Role
This role involves leading efforts to generate referrals, build strategic partnerships, and expand the agency's presence within the community. The Director connects with individuals, organizations, and healthcare partners to bring in new clients, caregiver transfers, and individuals in need of home care services. Responsibilities
- Actively network within the community to build relationships and generate referrals for home care services
- Establish and maintain partnerships with hospitals, rehab centers, senior centers, social workers, case managers, and community organizations
- Identify and connect with individuals who may qualify for home care services and guide them toward getting approved
- Recruit and build relationships with caregivers for potential transfers to the agency
- Coordinate and host lunch-and-learns, community presentations, and educational sessions
- Represent the agency at health fairs, community events, and outreach opportunities
- Secure opportunities for the agency to be present at local events, senior housing, and community programs
- Follow up with referral sources and maintain consistent communication to strengthen partnerships
- Track outreach efforts, referral sources, and results to ensure consistent growth
Requirements
- Strong networking and relationship-building skills
- Experience in healthcare, home care, Medicaid/waiver programs, or community outreach preferred
- Confident communicator with strong interpersonal and public speaking skills
- Self-motivated and able to work independently in the field
- Organized with the ability to track contacts, referrals, and follow-ups
- Sales mindset with a focus on growth and results
- Reliable transportation and willingness to travel locally
