About the role
A healthcare facility is seeking a Director of Case Management (RN) to provide leadership and strategic oversight for hospital case management operations. This on-site leadership role supports effective care coordination, utilization management, and discharge planning while collaborating with clinical and administrative teams to improve patient outcomes and continuity of care.
Responsibilities
- Provide leadership and direction for hospital case management operations.
- Oversee care coordination, utilization review, and discharge planning functions.
- Develop and implement departmental strategies to support quality patient care and operational goals.
- Lead, mentor, and support case management staff while promoting professional development and accountability.
- Collaborate with physicians, nursing leaders, and administrative teams to improve care transitions and patient outcomes.
- Ensure compliance with regulatory standards, organizational policies, and best practices.
- Monitor department performance and identify opportunities for process improvement.
- Support effective communication among interdisciplinary teams to promote coordinated patient care.
Requirements
- Current (or eligible) Montana RN license required.
- Minimum of one year of hospital case management leadership experience.
- Bachelor's degree required; advanced degree (MSN, Healthcare Administration, or MBA) strongly preferred.
- Preferred: Extensive hospital case management leadership experience.
