Job Description:

AHMG Diabetes and Endocrinology at Winter Garden

1223 Daniels Rd., Suite 2, Winter Garden, FL

Monday - Friday 8am - 2pm

will float to Clermont location 1-2 times per week, seeing patients on site and via telemedicine

  • Participates in special projects, community events, and healthcare professional education programs to increase diabetes awareness.
  • Completes documentation and record-keeping accurately and within expected time frames.
  • Provides ongoing therapeutic lifestyle coaching using approved resources and manages assigned caseloads.
  • Participates in practice changes resulting from performance improvement activities and supports departmental quality standards.
  • Serves as an advocate for care with community agencies, including pharmacies, insurance companies, and employers.
  • Educates patients and families about current research and assists with identifying and recruiting candidates for research studies.
  • Seeks professional growth opportunities and participates in community activities such as educational lectures and support groups.
  • Provides diabetes instruction in group and individual sessions according to American Diabetes Association standards and departmental policy.
  • Obtains individual patient assessments, identifies learning barriers, and provides appropriate resource referrals.
  • Establishes individualized learning goals and educational plans in collaboration with patients and families.
  • Prescribes appropriate nutrition plans based on patient needs and diagnoses.
  • Evaluates patients upon completion of educational plans and provides follow-up as needed.
  • Other duties as assigned.

Knowledge, Skills, and Abilities:

  • Advanced knowledge of diabetes concepts and skills including insulin pump therapy concepts as related to food intake. [Required]
  • Ability to provide teaching in all aspects of diabetes management and nutrition. [Required]
  • Ability to assess and interpret data relative to patient’s age and diagnosis. [Required]
  • Ability to provide comprehensive insulin pump training and glucose sensor training. [Required]
  • Ability to communicate effectively and organize daily activities. [Required]
  • Knowledge of computer programs such as Microsoft Outlook, Word, PowerPoint. [Required]
  • Ability to work with patients of various ages and backgrounds. [Required]
  • Ability to communicate fluently in English. [Required]
  • Knowledge of continuous glucose monitoring. [Preferred]
  • DiaWeb documentation system. [Preferred]
  • Bilingual: Spanish preferred. [Preferred]

  • Bachelor's [Required]

Work Experience:

  • 1+ experience in diabetes specialty [Required]
  • 1+ experience in the field [Required]

Pay Range:

$59,652.52 - $110,956.61
