Dementia Health Coach

About This Role

This role is for a dedicated Dementia Health Coach to join a multidisciplinary Dementia Care team as part of the expanding GUIDE Program. The coach provides compassionate peer support, helps families navigate complex care needs, and connects them with community resources, including respite care. The position focuses on care coordination, ensuring holistic, person-centered support for each patient and caregiver.

Responsibilities

• Provide coaching and emotional support, offering empathetic, patient-centered guidance to individuals with dementia and their caregivers, including support through grief, stress, and difficult conversations
• Connect families to resources, identifying and coordinating access to respite care, community programs, educational resources, and financial or social support
• Collaborate across disciplines, working closely with clinical and non-clinical team members to ensure integrated, comprehensive care planning
• Support care planning, assisting with ongoing care coordination, goal-setting, and monitoring of patient and caregiver needs
• Track progress and needs, maintaining organized, accurate documentation to support continuity of care and identify emerging challenges

Requirements

• Strong passion for dementia care and supporting caregivers with empathy, humility, and patience
• Communication skills that support tough conversations, grief support, and relationship-building over the phone
• Organizational abilities and self-motivation to manage multiple dyads in a remote environment
• Team-oriented mindset with the ability to rely on and collaborate with others
• Technology comfort using virtual care platforms and remote communication tools
• Situational awareness to navigate sensitive dynamics and maintain trust
• Experience in health coaching or related fields (caregiving, social work, behavioral health) or a willingness to grow through training
• Valid Driver’s License required

Nice to have:

• Bachelor’s Degree
• Health Coach Certification
• Two years of experience in a social setting or health care setting or equivalent education
• Bilingual
• Local community resident with knowledge of resources available in the community
• Proficient computer use
• Knowledge of Medical terminology

• Schedule: Full-time, 40 hours per week, Monday–Friday, 8:00 AM–4:30 PM
