About the role
Position Summary:
The System Pharmacy & Therapeutics (P&T) Coordinator is responsible for coordinating and operating the enterprise P&T governance framework across the health system, ensuring consistent, evidence-based, safe, and cost-effective medication-use decisions across acute and ambulatory settings (including infusion services).
This role serves as the central integrator for formulary governance, committee operations, and decision translation, working in close partnership with clinical leaders, pharmacy leadership, Clinical Quality Value Analysis (CQVA), Medical Staff leadership, and operational stakeholders. The role carries system-level accountability for governance mechanics, transparency, and follow-through.
Qualifications:
Required:
- PharmD (or BS Pharm with equivalent advanced experience).
- 5+ years of progressive clinical leadership.
- Active pharmacist license in New York or Pennsylvania (or eligible to obtain within 90 days).
- Significant experience in formulary management, medication-use policy, or enterprise clinical governance within a multi-hospital health system.
- Demonstrated ability to operate effectively in complex, matrixed environments with senior clinical and administrative leaders.

- Governance & Decision Integration: Ability to structure, coordinate, and maintain enterprise governance processes that translate multidisciplinary input into clear, actionable decisions.
- Analytical & Value Awareness: Ability to interpret and integrate clinical, operational, and economic considerations (including CQVA outputs) to support informed formulary and medication-use decisions.
- Operational Coordination: Skilled in managing complex, multi-committee workflows, standard processes, documentation, and follow-up across multiple entities.
- Collaboration & Influence: Effective partner with physicians, nursing, pharmacy leaders, CQVA, Informatics, and Medical Staff leadership; able to synthesize diverse perspectives and facilitate alignment.
- Communication & Transparency: Strong written and verbal communication skills; able to ensure decisions, rationale, and implications are clearly documented and accessible to stakeholders.

Coordinate the System Pharmacy & Therapeutics Committee.

Support the coordination of supporting subcommittees (including Infusion Center P&T, Ambulatory P&T, and Antimicrobial Stewardship reporting pathways).

Maintain the operating cadence, agendas, and standardized review processes that support effective formulary and medication-use governance across the system.

Ensure multidisciplinary input is appropriately captured, documented, and elevated through established governance pathways.

Formulary and Medication-Use Governance
-
Coordinate enterprise formulary review processes, including intake, documentation, and presentation of clinical, operational, and economic considerations.

Ensure consistent application of formulary, therapeutic interchange, and medication-use policies across inpatient, infusion, and ambulatory settings.

Support ongoing maintenance and evolution of the system formulary in alignment with evidence, safety considerations, and stewardship goals.

Decision Routing, Documentation, and Follow-Through

Ensure P&T recommendations, minutes, and supporting materials are routed through the appropriate medical staff approval pathways (e.g., MEC) prior to implementation.

Translate committee decisions into clear, actionable outputs for downstream implementation and communication.

Track decisions, conditions, and follow-up actions to support accountability and transparency.

System Communication and Stakeholder Engagement

Maintain systemwide P&T documentation and communication infrastructure.

Coordinate dissemination of formulary and medication-use decisions in partnership with Medical Staff, Pharmacy, Quality (Performance & Outcomes), and operational leaders.

Support structured provider input pathways and escalation processes, particularly for high-impact or cross-setting formulary changes.

Cross-Functional Integration
This role facilitates governance and decision translation; execution activities are completed by the appropriate accountable owners.
- Partner with operational stakeholders to ensure P&T decisions are implementable and aligned with system priorities through established governance pathways.
- Coordinate structured handoffs of approved P&T decisions to the appropriate accountable owners (individuals or teams) for implementation (e.g., build/configuration, policy/procedure updates, education/communication, monitoring), and track follow-through through established governance documentation when implementation activities are required.
- Engage impacted stakeholders upstream for high-impact formulary changes so committee recommendations reflect clinical, operational, and economic considerations across entities.
- Coordinate with related governance bodies to ensure alignment, manage interdependence, and avoid duplication across committees and workgroups.
- Coordinate policy management and collaborate with policy owners to ensure required reviews are completed timely and needed revisions are completed and brought through the P&T process for approval.
