Community Outreach Coordinator

About This Role

Connecting with local groups and raising awareness of available services is the core of this role, which supports a clinic treating a diverse population of patients with substance use disorders, primary care, infectious disease, and mental health needs. This position suits a kind, optimistic, and compassionate person who wants to help build connections to fight the opioid crisis in the area.

Responsibilities

• Develop and work with agencies and service providers to facilitate coordination and information sharing; provide support services as applicable
• Participate in health fairs, community events, and other events where the target population gathers
• Maintain a calendar of outreach activities, including community events, workshops, appearances, and other communication opportunities
• Establish and maintain positive working relationships with current and potential referral sources
• Maintain comprehensive working knowledge of the clinic’s markets including government agencies, major payor groups, key referral sources, and competitor’s market positioning
• Maintain comprehensive working knowledge of community resources and assist patients in accessing community resources
• Participate in staff meetings and work with other administrative staff as required
• Maintain professional and congenial attitude toward coworkers, patients, agencies, and members in the community
• Develop, create, and maintain a resource file of available social service agencies and assistance providers in the target area for use in referring patients for needed services
• Use appropriate technology tools to accomplish job functions; understand and utilize available technology as customer service, communications, and data-gathering tools
• React to frequent changes in duties and volume of work
