Role Overview This position provides a critical link for referring physicians, offering 24/7 support for patient admissions, consultations, transportation, appointment scheduling, and information requests. It involves a hybrid work model, primarily during evening and night shifts, with weekend and holiday rotations.
Responsibilities
- Deliver patient and family-centered care, including education on managing illness, medications, and post-treatment needs.
- Participate in activities promoting patient safety, quality, and compliance, while engaging in ongoing professional development.
- Develop, implement, and document individual care plans collaboratively, ensuring clear communication across care teams.
- Assess physical, psychosocial, and emotional needs, respecting patient preferences and cultural considerations.
- Work in a safety-sensitive environment, maintaining constant alertness and adherence to safety protocols.
Qualifications & Skills
- Minimum education: Nursing Diploma or Associate's Degree in Nursing; preferred Bachelor's Degree.
- Licensure: Active RN license.
- Experience: At least 2 years in pediatric ICU or ED nursing preferred.
- Additional skills: Computer proficiency and typing skills.
