About the role
Clinical Care Manager (RN) – Home Health Lead care coordination, clinical decision-making, and patient advocacy in a dynamic home health environment. This role requires a dedicated RN to oversee the full patient journey, ensuring care is coordinated, compliant, and tailored to individual needs. Responsibilities include managing physician orders, documentation, and workflows within the EMR, supporting timely admissions, conducting patient assessments, reviewing clinical reports, leading case conferences, and discharge planning. The manager acts as a liaison among physicians, patients, families, and internal teams, and may support in-home visits as necessary.
Qualifications
- Active RN license; BSN preferred
- Home health experience and clinical leadership background preferred
- Strong clinical judgment, organizational skills, and ability to work independently
- Effective communication and collaboration skills
