About This Role
Caring for individuals facing challenges such as addiction, behavioral health issues, and social welfare needs is the core of this Case Manager role. The position requires strong crisis intervention skills and the ability to work collaboratively with clients to develop effective support plans, guiding them towards recovery and self-sufficiency.
Responsibilities
- Conduct comprehensive assessments of clients' needs and develop individualized service plans
- Provide crisis intervention and management to clients in urgent situations
- Offer addiction counseling and support to individuals struggling with substance use disorders
- Collaborate with healthcare providers, social services, and community resources to ensure holistic care
- Utilize motivational interviewing techniques to empower clients in their recovery process
- Supervise and mentor junior staff or interns as needed, fostering a supportive learning environment
- Assist clients in navigating Section 8 housing applications and child welfare services
- Facilitate workforce development programs aimed at enhancing clients' employability skills
- Maintain accurate documentation of client progress and case notes in compliance with organizational policies
Requirements
- Bachelor’s degree in Social Work, Psychology, or a related field, or a High School Diploma and relevant experience
- Proven experience in crisis intervention and management within behavioral health settings
- Familiarity with addiction counseling techniques and resources available for substance use treatment
- Strong understanding of child welfare systems and Section 8 housing regulations
- Excellent communication skills, both verbal and written, with the ability to build rapport with diverse populations
- Ability to work independently as well as part of a collaborative team environment
Nice to have:
- Master’s degree
- Experience in supervising staff or interns
- Knowledge of Medicare processes and resources
