About the role
Role Overview:
The Case Manager Social Worker (LBSW ONLY) provides leadership within the healthcare team by coordinating post-acute care services and delivering psychosocial support to patients and families.
Key Responsibilities
- Assess patient and family needs using diverse information sources to develop individualized care plans.
- Participate in interdisciplinary collaborations to identify and address post-acute care requirements, incorporating social, cultural, emotional, and spiritual factors.
- Offer crisis intervention and counseling to support emotional adjustment to medical episodes.
- Document assessments, intervention plans, and outcomes per departmental and hospital policies.
- Coordinate with care teams, payors, and resources to ensure timely and appropriate services, advocating for efficient discharge planning.
- Educate staff on bed utilization, length of stay, and payment processes.
- Lead departmental initiatives, participate in staff development, and engage in quality improvement projects.
- Maintain effective communication and uphold professional social work ethics.
Qualifications & Skills
- Bachelor’s in Social Work and LBSW license in MD required.
- Minimum of 3 years of social work experience; 5+ years preferred.
- Strong assessment, crisis intervention, documentation, and interdisciplinary collaboration skills.
- Ability to prioritize workload, demonstrate professionalism, and contribute to team efforts.
