NOW HIRING: Remote RNs & LVNs (California)
Schedule: Mon–Fri | 8am/8:30am – 5pm/5:30pm PST (40 hrs/wk)
Location: 100% Remote (Must reside in California)
This is a 4 month contract role.
Overview: We are seeking California-licensed RNs and LVNs for a specialized SNP compliance project. Working as a collaborative team, nurses partner on member assessments, care plan finalization, and SBAR preparation for ICT meetings.
Registered Nurse (RN) – Care Coordination Lead
- Role: Lead clinical oversight, finalize ICPs, review LVN-prepared SBARs, and ensure CMS compliance.
- Requirements: Active CA RN License | 2+ yrs RN experience.
- Preferred: 2+ yrs in Care/Case/Utilization Management, D-SNP knowledge, LVN delegation experience.
Licensed Vocational Nurse (LVN) – Care Coordination Support
- Role: Conduct targeted data collection, chart reviews, draft ICPs, and prepare SBAR summaries under RN guidance.
- Requirements: Active CA LVN License | 1+ yr LVN experience.
- Preferred: 1+ yr in Care Coordination, Managed Care, or Ambulatory Care; chart review experience.
