Position Overview:
The Care Manager RN provides complex care management and coordination for foster care members enrolled in Medicaid/KanCare. The role focuses on clinical assessments, individualized care plans, member education, and coordination of medical, behavioral health, and community-based services.

Key Responsibilities:
- Conduct member assessments, including HRAs, telephonic outreach, and in-person/home visits.
- Complete required 48-hour visits following placement changes and 7-day IDT meetings.
- Develop, monitor, and update individualized care plans/Plans of Service.
- Coordinate services with members, families, providers, DCF, schools, foster families, and community resources.
- Identify barriers to care and connect members with appropriate healthcare and HCBS resources.
- Monitor member status, care-plan progress, and changing needs.
- Provide education regarding healthcare, treatment options, referrals, and benefits.
- Maintain accurate documentation and ensure compliance with Medicaid, state, and contractual requirements.
- Manage assigned caseload and meet required visit, outreach, and documentation timelines.
Requirements:
- Degree from an accredited School of Nursing; behavioral health/social work education may also be considered.
- Active RN license; LMSW/LCSW may be considered based on requirements.
- RN with behavioral health experience or social work experience with 1–2 years of child welfare experience.
- Experience with foster care/child welfare and care coordination required.
- Strong clinical assessment, documentation, communication, and organizational skills.
- HCBS waiver experience preferred.
- Valid driver's license required due to field travel.
