Care Manager (RN) – Remote
Duration: 6 months (with a chance to extend or go permanent)
Location: Remote, but may require occasional in-person visits in the NE region/ Near Cuyahoga
Schedule: Monday – Friday, 8 AM – 5 PM (No overtime)
About the Role:
We’re looking for an RN Care Manager to support members new to the health plan . You’ll help coordinate care, create personalized care plans, and guide members and their families to better health outcomes. This role mainly involves phone-based care management , but some in-person visits may be needed for specific assessments.
What You Need:
✔️ RN License
✔️ Experience in case management, care transitions, and treatment planning
✔️ Strong documentation and assessment skills
Active Ohio RN License
- 2–4 years of nursing experience (case management/home health preferred)
Strong computer proficiency – Microsoft Office Suite (Excel, Word, Teams, Outlook, OneNote, OneDrive, PowerPoint) and ability to learn new systems
Independent critical thinker – Strong problem-solving, communication skills, ability to manage change and work autonomously
