About the role
Role: RN Care Coordinator responsible for assessing, planning, and coordinating patient care during transitions, ensuring efficient discharge and care progression.
Responsibilities: Conduct psychosocial and functional assessments, collaborate with interdisciplinary teams, develop individualized discharge plans, manage barriers to discharge, facilitate communication, and document care activities. Support post-acute referrals and ensure compliance with regulations, contributing to patient-centered care and resource utilization.
Qualifications & Skills: Nursing degree (Associates, Diploma, or Bachelors preferred), current RN licensure (single-state or multi-state), BLS certification, minimum 1 year acute care experience, excellent communication, organizational, and problem-solving skills. Ability to work collaboratively in a fast-paced environment and handle stress effectively.
