About the role
Role: The Care Coordinator RN (CC RN) assesses and manages patients' transitional care needs, coordinating across care settings to ensure smooth discharge and ongoing support.
Responsibilities: Conduct psychosocial and functional assessments, develop individualized discharge plans, facilitate interdisciplinary rounds, and collaborate with physicians and care teams to optimize care progression. Manage discharge processes, identify barriers, and coordinate post-acute referrals. Document care activities accurately, track avoidable days, and support professional development.
Qualifications & Skills: Requires an RN license (single or multi-state), BLS certification, at least 1 year of acute care nursing experience, and excellent communication, organizational, and problem-solving skills. Knowledge of healthcare regulations and community resources is essential.
Key Duties: Ensure timely care transitions, promote resource utilization, and advocate for patient needs, working proactively to resolve delays and barriers to discharge.
