About the role
Role: Provides care coordination for members with special health care needs through face-to-face and in-home assessments, developing client-focused service plans collaboratively with healthcare providers.
Responsibilities
- Conducts comprehensive assessments and creates service plans within specified timeframes.
- Maintains accurate documentation in electronic health records, tracking assessments, care, and progress.
- Identifies resources, makes referrals for dental, behavioral, social, and protective services, and assists with community-based services.
- Serves as a resource for members, families, providers, and community agencies regarding healthcare and placement options.
- Coordinates with care team members to navigate services, supplies, and equipment for members.
- Provides social work consultation and develops community resources.
Qualifications
- Master’s degree in social work or six years of related experience.
- Licensed Clinical Social Worker or Licensed Social Worker in Hawaii.
- One year of social work experience required; care coordination experience preferred.
- Basic Life Support certification.
Skills: Excellent communication, problem-solving, teamwork, documentation accuracy, customer service, interpersonal skills, adaptability, and conflict resolution. Experience with community networking and healthcare is preferred.
