About the role
Role Overview: The RN will provide advanced clinical home care services, including patient assessment, medication management, and family education, to support recovery at home.
Key Responsibilities
- Create and update comprehensive, patient-centered home health care plans in collaboration with physicians.
- Case manage assigned patients to achieve positive clinical outcomes and reduce rehospitalization risks.
- Coordinate and evaluate individual care plans, ensuring effective communication with family and professional caregivers.
- Support agency financial goals through quality care delivery.
- Demonstrate expertise in community health nursing practices.
Qualifications And Skills
- Current NYS RN license; Bachelor's degree in nursing preferred.
- Minimum one year of acute medical-surgical nursing experience.
- Excellent clinical assessment, problem-solving, and communication skills.
- Ability to work independently and within an integrated team.
- Driver's license and car required; bilingual in English/Spanish preferred.
