About the role
Role: The Care Coordinator RN (CC RN) assesses patients' transitional care needs, coordinates care across the continuum, and collaborates with patients, families, and care teams to ensure effective discharge planning and care progression.
Responsibilities: Conduct psychosocial and functional assessments, develop individualized discharge plans, facilitate interdisciplinary rounds, address barriers to care, and manage post-acute referrals. Ensures accurate documentation, supports care progression, and promotes resource utilization.
Qualifications: Requires an Associate’s or Bachelor’s degree in Nursing, RN license (single or multi-state), BLS certification, and at least 1 year of acute care nursing experience.
Skills: Excellent communication, assessment, organizational, and problem-solving abilities; ability to work under stress, handle change, and collaborate effectively with care teams and families. Knowledge of healthcare regulations and community resources is essential.
