Care Coordinator

About This Role

This full-time Care Coordinator position, based at West Parry Sound Health Centre, involves assessing patient care needs and eligibility, providing access and referrals to community services, and engaging with patients, caregivers, and other healthcare practitioners. The role is ideal for an experienced registered nurse, physiotherapist, occupational therapist, or social worker who is passionate about providing connected, accessible, patient-centred care.

Responsibilities

• Assess care needs and determine eligibility for services in collaboration with patients and their families
• Develop individual care and service plans
• Link patients with service providers
• Coordinate and monitor care plan delivery
• Establish a helping relationship with patients and their families
• Balance patient needs and choices with available resources, ensuring patients’ values and preferences are respected
• Initial assignment to Hospital, with possible assignment changes within the occupation based on operational needs

Requirements

• Membership, in good standing, with the applicable regulatory body: College of Nurses of Ontario, College of Physiotherapists of Ontario, College of Occupational Therapists of Ontario, or Ontario College of Social Workers and Social Service Workers
• Recent experience in community health or a related field
• Knowledge of the healthcare delivery system and community resources
• Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
• Effective time management, prioritization, and organizational skills, with the ability to work independently and cooperatively in a busy multidisciplinary environment
• Established ability to accurately complete required documentation, reports, and forms
• A valid driver’s licence and access to a reliable vehicle
• Proficient in a Windows environment
• Mandatory COVID-19 vaccination policy; proof of COVID-19 vaccination status required prior to start date
• Advance oral and written proficiency in English

Nice to have:

• Experience working with diverse patient groups, e.g., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatrics
• Case management experience or recent related community experience
• Ability to speak French or another second language
