Responsibilities:
- Coordinate case management and care planning with multidisciplinary teams.
- Assess patients' medical, psychosocial, and functional needs to support individualized care plans.
- Identify care gaps and facilitate appropriate interventions, referrals, and community support services.
- Promote healthy lifestyle changes and support advance care planning discussions with patients and caregivers.
- Monitor patients' progress and ensure smooth care transitions through timely follow-up.
- Manage and triage referrals for transitional and community care services.
- Maintain accurate documentation, referral records, and operational databases.
- Collaborate with internal teams and external partners to improve care coordination and service delivery.
- Provide guidance and support to junior care coordination staff.
- Participate in quality improvement, research, education, professional development, and community outreach activities.
Requirements:
- Minimum 5 years of experience in the healthcare industry.
- Experience collaborating with clinicians and multidisciplinary care teams to develop or implement healthcare programmes and projects is an added advantage.
- Knowledge of specialized medical and surgical services, including rehabilitation and psychological medicine, is an added advantage.
- Experience in home-based healthcare, transitional care, or community care settings is an added advantage.
- Strong interpersonal, stakeholder management, coordination, and problem-solving skills.
- Ability to work independently and effectively in a multidisciplinary environment.
- Good verbal and written communication skills.
- Proficiency in Microsoft Office applications; familiarity with healthcare information systems is an added advantage.
