About the role
We are currently seeking a dedicated and Bilingual (Spanish/English) Patient Intake Coordinator to join our team at our Bronx location. As a Patient Intake Coordinator, you will play a crucial role in facilitating the enrollment process for individuals seeking home care services.
Responsibilities:
- Explain program benefits, and conduct initial eligibility (Medicaid and Managed Long-Term Care) reviews and screenings for individuals interested in participating for home care services.
- Conduct outbound calls to an assigned census of 40-50 active patient cases and answer incoming calls with a goal of 30+ monthly conversations converted into qualified leads.
- Coordinate and manage the intake process, including gathering necessary documentation and information.
- Collaborate with healthcare professionals, caregivers, and clients to ensure smooth onboarding and enrollment procedures.
- Maintain accurate and confidential records of all intake activities and participant information.
- Provide exceptional customer service by educating clients and caregivers on program guidelines and procedures and addressing inquiries, and concerns, throughout the intake process.
Requirements:
- Fluency in both English and Spanish (written and verbal) is essential.
- Experience in a similar role in Intake or Onboarding home care clients.
- Strong organizational skills with attention to detail.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively in a team-oriented environment.
- Proficiency in basic computer applications, (Salesforce a plus) and data entry.
