As an Assistant Nurse Clinician [Home Care], you will be responsible for the following duties:
- Provide clinical supervision, mentorship and coaching to nursing staff to ensure the delivery of safe, high-quality and evidence-based patient care.
- Support the triage and prioritisation of referrals, and contribute to case discussions to promote timely and appropriate care planning.
- Participate in departmental and multidisciplinary meetings, while providing support with operational and administrative responsibilities.
- Manage clinical escalations raised by nursing staff by providing appropriate clinical guidance, advice and support.
- Monitor nursing practices, workload allocation and resource utilisation, and identify opportunities to improve efficiency and service delivery.
- Contribute to the development, review and implementation of community nursing guidelines, clinical protocols and standard operating procedures.
- Support staff performance management by identifying training and development needs, facilitating competency enhancement and encouraging continuous professional development.
- Participate in quality improvement activities, clinical audits and service-related projects to promote positive clinical outcomes and enhance patient safety.
- Undertake any other duties and responsibilities as assigned by the reporting officer.
Requirements:
- Possess a Degree in Nursing with a valid full registration with Singapore Nursing Board (SNB).
