About the role
Position: Afterhours Registered Nurse Join a dedicated hospice care team providing comprehensive afterhours services, including night and weekend care.
Key Responsibilities
- Triaging patient needs via phone and responding to urgent crisis calls
- Performing initial nursing evaluations to determine hospice eligibility within 48 hours of referral
- Conducting afterhours visits such as admissions, death, recertification, and routine assessments
- Assessing and reassessing patients’ physical, psychosocial, emotional, and environmental needs at least every 15 days
- Documenting patient issues, goals, interventions, and responses to care
The role requires current RN licensure, a valid driver’s license, vehicle insurance, and reliable transportation.
