Role Overview This position provides clinical leadership, managing inter-professional teams and delivering advanced nursing care in compliance with state, federal, and national standards. The role involves evaluating patient responses, leading care teams, and developing individualized treatment plans across diverse settings, including virtual, home, and facility-based care.
Key Responsibilities
- Manage full scope of advanced nursing practice for targeted populations, including assessments, interventions, and care revisions.
- Lead interdisciplinary team rounds and meetings, ensuring quality outcomes and compliance with care standards.
- Provide comprehensive clinical services such as post-discharge visits, assessments, palliative care, and urgent interventions.
- Collaborate with patients, families, and healthcare providers for assessment, planning, education, and care management.
- Ensure documentation accuracy, billing compliance, and participate in quality improvement initiatives.
- Perform procedures within collaborative practice agreements and act as a preceptor or peer reviewer as needed.
- Support continuous professional development, maintain licensure, and participate in special projects and committees.
Qualifications & Skills
- Current RN and Nurse Practitioner licenses in New York; certification by ANCC or similar body.
- Master’s degree in Nursing; psychiatric-mental health certification for Psychiatric Nurse Practitioners.
- Minimum two years of relevant clinical experience; home care or managerial experience preferred.
- Strong knowledge of quality measures, coding, and documentation; bilingual skills may be required.
Additional Details: Compensation is per visit and hourly for training; pay range from USD $58.30 to USD $77.72 per hour. The role involves providing care across various environments, including virtual and home settings, with opportunities for professional growth and participation in quality improvement initiatives.
