We offer full service home health care to our clients in the comfort of their homes, utilizing the most caring, competent and reliable professionals. Our collaborative team approach strives to meet and exceed standards for licensed and certified home health care. All patients must be home bound. Referrals are accepted from physicians, community service organizations, hospitals, case managers and patients. Patients are staffed by qualified licensed and certified professionals- registered nurse, licensed vocation nurse (LVN), physical therapist, occupational therapist, speech therapist and medical social worker. Services provided includes: skilled nursing, pediatrics, home health aides, rehabilitation services, medical social workers, IV infusion therapy and primary home care. Counties served: Harris, Austin, Brazoria, Liberty, Matagorda, Montgomery, Walker, Waller, Wharton, Washington, Chambers, San Jacinto, Colorado, Fort Bend, Galveston.
