Tampa General Hospital (TGH) is partnering with the Visiting Nurse Association (VNA) to form TGH Home Care powered by VNA of Florida. This new entity will offer a full range of home care services for TGH patients and the wider community. The goal is to allow patients to recover in the comfort of their homes while remaining within TGH's network of care, ensuring a seamless and safer transition. This partnership combines the resources of an academic medical center with an experienced home care agency to improve quality, outcomes, and the overall patient experience. Home care services include personal care and skilled nursing, such as home health aides, skilled nursing, rehabilitation, chronic disease management, and medical social services. The VNA has been providing home health services in Florida since 1976, with a long-standing expertise in serving patients with complex needs. TGH Home Care powered by VNA of Florida ensures continuous care coordination through electronic medical record integration and offers access to nurse navigators for ongoing support. The service is available to patients in Hillsborough, Pasco, Pinellas, Polk, Manatee, Hardee, and Highlands counties, regardless of the hospital they are discharged from. The VNA has seven locations across Florida to support patients statewide. Services began June 20.
