The EPSM is located in La Roche-sur-Foron, a small medieval town of 10,000 inhabitants at an altitude of 548 meters, near Annecy, Geneva, and Chamonix. It is situated in an area with one of the highest concentrations of small and medium-sized enterprises in France. The EPSM was created in 1975, initially attached to the Reignier retirement home, and became autonomous in 1978. Since June 2012, the structure of the La Roche-sur-Foron site has been situated in a privileged, rural setting, open to the town. In 2020, the psychiatry sectors of Chablais, centered around the Thonon and Evian facilities, were attached to the establishment, which then became EPSM 74. Today, the Public Mental Health Establishment can accommodate 208 adult patients in full hospitalization, across the La Roche-sur-Foron and Thonon-les-Bains sites. Four adult sectors are equipped with Medical-Psychological Centers (CMP), Partial Day Care Centers (CATTP), and Day Hospitals (HdJ) in the towns of Vétraz-Monthoux, La Roche-sur-Foron, Bonneville, Cluses, Sallanches, St Julien-en-Genevois, Thonon-les-Bains, and Evian-les-Bains. The specialization of certain units aligns with the objectives of the establishment's project: a unit for gerontopsychiatry, a unit for psychosocial rehabilitation, a unit for long-term patients, and an interdepartmental Psychiatric Intensive Care Unit (Isère, Savoie, Haute-Savoie) opened in the fall of 2012. Child and adolescent psychiatry services are distributed across the towns of Vétraz-Monthoux, Saint Julien-en-Genevois, La Roche-sur-Foron, Cluses, Sallanches, Thonon-les-Bains, and Evian-les-Bains.
