Primary PartnerCare® is a physician-driven organization with doctors in Nassau, Suffolk, Queens, and New York Counties. We were founded on the belief that private practice primary care doctors in partnership with their patients produce optimal health. Established in 2012 to help independent physicians meet the demands of healthcare reform, we provide tools and resources for doctors to remain independent and thrive. The mission of Primary PartnerCare® is to preserve and enhance the private practice of medicine and provide a simplified pathway for independent physicians to participate in accountable care payment and delivery models . Essential to our mission is the coordination of our patients’ care, open communication, excellent clinical and service quality, accountability, and continuous improvement and innovation.
