Hospitalisation à domicile Santé Val D'oise provides care for patients at home and in medico-social structures (EHPAD, etc.) within the DAC 95 Ouest territory. Subject to the same regulatory obligations as a hospital structure, the establishment ensures continuity of care 24 hours a day, 7 days a week. Admission to Home Hospitalization is subject to a medical prescription from a treating physician or a hospital physician. Each admission request is evaluated by a HAD nurse who visits the patient's home, the medico-social establishment where they reside, or a healthcare facility if they are hospitalized. Our main modes of care are: - Palliative care - Complex wound care (including TPN) - Parenteral/enteral nutrition - Intravenous treatments - Post-chemotherapy monitoring - Heavy nursing care - Rehabilitation / Re-adaptation
