COSSMA Inc. is a non-profit organization that has been providing health services to the communities of the Central and Eastern Area of Puerto Rico for 25 years. Mission At COSSMA, we are leaders in providing the highest quality primary health services with expert staff committed to the health of the population. Description COSSMA was founded in 1987 as a private non-profit corporation. In its beginnings, in the 1960s, it provided health services exclusively to migrants, agricultural workers, and their families from the towns in the Caguas health region: Cidra, Caguas, Aguas Buenas, Cayey, Aibonito, Gurabo, San Lorenzo, Juncos, Las Piedras, Maunabo, Naguabo, Yabucoa, and Humacao. Starting in 1995, COSSMA expanded its health services to the general public, responding to the new trends in coordinated medical care implemented through the Health Reform. General Information COSSMA offers its services to the general population as follows: - Reform beneficiaries by selecting us as their primary center. - Migrant or farmer from the towns in the Caguas region. - Person without medical insurance requesting an appointment for orientation on our special rates. - Persons with medical insurance requesting an appointment. Currently, COSSMA is a leading corporation in the health field, providing and giving access to all health services contemplated by the reform, namely: Medical Services (Family Medicine, Pediatrics, Gynecology/Obstetrics) - Family Planning - Vaccination - Home Visits - Health Certificates Dental (Diagnosis and Treatment) Pharmacy Laboratory Referrals Nutrition Social Work Mental Health Health Education Community Impact - HIV/AIDS Prevention - Chronic Disease Management - Health Promotion - Disease Prevention
