Chronic Care Staffing, LLC (CCS) was founded in 2015, the same year Medicare’s Chronic Care Management CPT 99490 program began. Our headquarters are in Charleston, SC. Chronic Care Staffing has compiled an array of clients ranging from small practices to large health systems. Our clinical team has developed industry-leading documentation, population health initiatives, and Value-Based Reimbursement (VBR) calling protocols to enhance future reimbursement. We offer an industry-leading clinical approach to patient health management via Chronic Care Management (CCM), Annual Wellness Visits (AWV), Behavioral Health Integration (BHI), and Transition of Care Management (TCM) programs. We have a collaborative work style with the ability to work directly in your practice EMR. Our program is designed to significantly improve HEDIS scores and maximize MIPS and MACRA valuations. Our outsourced model is proven to be more effective than in-house management. We provide access to a network of highly qualified nurses across the US via our sister company Millenia Medical Staffing.
