Chosen Family is an inclusive, trauma informed NDIS provider delivering person centred support across social and community participation, support coordination, and psychology services. We work alongside participants to understand their goals, experiences, and everyday needs, and design support that reflects real life, not just a plan on paper. Our approach is practical, flexible, and grounded in building meaningful outcomes over time. Our services include community participation, daily living support, support coordination, and access to psychology services. We support individuals to build independence, strengthen relationships, and reconnect with their communities in ways that feel safe, comfortable, and aligned with who they are. Chosen Family operates through a wraparound support model, where support workers, coordinators, and clinicians work together to provide consistent and connected care. This ensures participants are supported across all areas of life, rather than navigating services in isolation. We have a strong focus on supporting individuals with complex needs, including those experiencing mental health challenges, trauma, or social isolation. Our team takes a respectful and understanding approach, ensuring support is always person centred and identity affirming. We work closely with support coordinators, plan managers, allied health professionals, and community organisations to build strong referral pathways and collaborative outcomes.
