Capital Post-Acute is a 121-bed skilled nursing and rehabilitation facility with a 24-bed sub-acute care unit serving the South Sacramento community. We provide short-term rehabilitation, skilled nursing, long-term care, and specialized sub-acute services in a compassionate, patient-centered environment where every individual is treated with dignity and respect.
