Our areas of service include: Home Health Care Home health care is for those who are recovering from surgery or illness, or those living with a chronic condition, such as diabetes, Parkinson’s or Alzheimer’s. Our team of nurses, therapists, social workers and other skilled caregivers work with your private physician to help ensure you fully recover from illness or surgery or are able to manage and continue to live well with a chronic illness. Your home health team will work with you to help you reach your individual goals, while helping to maximize your health, function and independence. Hospice Care When a loved one is nearing the end of life, Blize’s hospice care helps both the patient and their family recognize the beauty of life and helps them make the most of every moment. Hospice care is all about celebrating life and making the end-of-life transition one of comfort, peace and closure. Private Duty Homecare According the AARP, nearly 90 percent of seniors want to stay in their homes as they age. Private duty homecare allows seniors to age in place by providing a variety of nonmedical services that allows them to continue living as independently as possible, while enjoying the things that make life worthwhile.
