Albany Area Primary Health Care (AAPHC) is one of the largest physician groups in Albany, Georgia and one of the state’s largest Community Health Centers. Our organization provides high-quality health care to residents across 11 Southwest Georgia counties. AAPHC is comprised of over 30 locations that include clinical sites, mobile access units, school-based health centers, pharmacies and administrative offices. Our physician group includes over 90 providers that specialize in Aesthetics, Behavioral Health, Dentistry, Family Medicine, Internal Medicine, Pediatrics, Obstetrics and Gynecology, Nurse-Midwifery, Podiatry, Optometry and Ophthalmology. As a Level III Patient Centered Medical Home, our focus remains on raising the quality of care for our patients while reducing a patient’s overall costs. Our quality measures, and focus on providing exceptional patient experience, have garnered AAPHC national recognition and awards from the Health Resources and Services Administration (HRSA). AAPHC was nationally recognized with the Innovation & Quality Award for our At-Home Care Program to keep patients safe during the COVID-19 pandemic.
